Private TSV Intake Referrals/Requests: Please ask the party who
is considering our service to go to the website and download and
complete the "Private Intake" form. The completed form
and a screening fee of $30 can be forwarded to our Berkeley offices
via U.S. post or fax (510) 883-9314 to the attention of Rhonda M.
Lawrence, MFT. Both parties to the case need to submit separate,
completed intake forms and screening fees. I will be making arrangements
for any ongoing intakes that I'm working on during my absence. Please
do NOT tell anyone that "we can or will take their case."
Please just assure them that we have a screening process and completing
the intake documents is the way to begin this process. If anyone
has any questions about availability of clinicians or facilities,
assure them that these logistical details will be worked out when
and if their case is accepted. We no longer accept cases without
following this careful screening process.
